Change Social Care Solutions is a London Care Agency, committed to delivering an excellent and professional service to individuals within the community, through a range of home care services. We also endeavour to assist individuals to participate and promote their independence through extensive work using a holistic and person-centred approach. Whilst ensuring that they are accessing and receiving the appropriate services to enable them to make informed choices/decisions regarding their care.
Through the range of inclusive services which are provided to individuals it will enable them to regain their independence, whilst supporting them to live as independently as possible through a comprehensive Care Plan Approach (CPA). This will be conducted with customers, their carers and or with social workers, Probation Officers and/or other professionals who are involved in their care to ascertain the appropriate support needs.
We will identify your underlining issues by using a range of holistic approaches to assess your Physical, Intellectual, Emotional, Spiritual and Social needs which will effectively address your behaviour and change your mindset to enable you to live and maintain a much higher quality of life through our care services.
Through our professional care services across London, individuals will be able to create and develop life skills which they can implement thus changing their behaviour and life styles. These holistic methods can also be useful to unlock or address those who have been diagnosed with Alzheimer, Dementia, learning or physical disability or individuals with low to high support needs. Change Social Care Solutions provides a range of services under one umbrella a one stop shop service/approach.

Anne has over 8 years’ experience in care sector, she began her career as a care assistant working with vulnerable adults, and loved every minute of her work, after 6 years of working as care assistant she become a Care Coordinator and later a Service Care Manager, her current position.
